Description
Emerging from the lateral aspect of the pons, the trigeminal nerve (CN V) is separated into a larger sensory root positioned superiorly and a smaller motor root lying inferior and slightly medial before it passes toward Meckel’s cave. The sensory root expands into the semilunar (trigeminal, Gasserian) ganglion within Meckel’s cave, while the motor fibers course beneath the ganglion to join the mandibular division (V3). Anterior and lateral to the ganglion, the three major divisions are implied: ophthalmic (V1) superiorly, maxillary (V2) anteromedially, and mandibular (V3) inferiorly. Orientation of the sensory and motor roots matters when you plan procedures around the trigeminal cistern and ganglion. Percutaneous approaches for trigeminal neuralgia, including radiofrequency rhizotomy, glycerol rhizolysis, or balloon compression, target the sensory root and ganglion via the foramen ovale, and the inferior position of the motor root explains how masseter weakness can occur if instrumentation strays caudally. The relationship to Meckel’s cave also frames imaging interpretation, since meningioma or schwannoma in this compartment can widen the trigeminal cistern and displace the roots before producing facial hypoesthesia or loss of the corneal reflex.
