Description

Arising from the anterolateral surface of the pons, the large sensory root of the trigeminal nerve (CN V) courses anteriorly and laterally toward the petrous apex, where it expands into the trigeminal (semilunar, Gasserian) ganglion within Meckel’s cave. Superior and slightly medial to it, the smaller motor root runs parallel before joining the mandibular division (V3) distal to the ganglion. From the ganglion, the ophthalmic (V1) passes anterosuperiorly toward the cavernous sinus and superior orbital fissure, the maxillary (V2) tracks anteriorly toward the foramen rotundum, and the mandibular (V3) descends toward the foramen ovale. The surrounding brainstem anatomy frames the exit zone against the pons, with adjacent cranial nerves emerging at the pontomedullary junction and midbrain for orientation. Big nerve, short course. This anterior brainstem perspective matters because the trigeminal root entry zone at the lateral pons is the usual site of neurovascular compression in classic trigeminal neuralgia, most often by the superior cerebellar artery, and the relationship between the sensory root, petrous apex, and Meckel’s cave guides both diagnosis and operative planning.
