Description
Arising from the maxillary division (V2) of the trigeminal nerve in the pterygopalatine fossa, the zygomatic nerve enters the orbit through the inferior orbital fissure and divides into zygomaticofacial and zygomaticotemporal branches. In lateral profile, the zygomaticotemporal nerve tracks along the lateral orbital wall, then passes through the zygomaticotemporal foramen of the zygomatic bone to reach the temporal fossa. Its cutaneous twigs spread superiorly and posteriorly over the anterior temple, lying deep to the temporalis fascia before becoming subcutaneous. Small nerve. Clear course. Clinically, the zygomaticotemporal nerve is a frequent culprit in temporal and frontotemporal neuralgia and is a recognized trigger site in some migraine patterns, where local anesthetic block at the zygomaticotemporal foramen or within the temporal fossa can be both diagnostic and therapeutic. Surgeons also care about it during lateral orbitotomy, zygomatic fracture repair, and coronal or temporal approaches, since traction or transection can leave a patch of numbness over the temple and can contribute to postoperative dysesthesia. Its communicating branch to the lacrimal nerve (V1) is the anatomic basis for parasympathetic secretomotor fibers reaching the lacrimal gland from V2 via the pterygopalatine ganglion. Use this asset for gross anatomy teaching on the trigeminal nerve, for head and neck blocks and pain-medicine references, or for operative atlases covering the lateral orbital wall, zygoma, and temporal fossa.
