Description
Arising from the trigeminal (semilunar) ganglion, the maxillary nerve (CN V2) is shown as the middle division of the trigeminal nerve, positioned inferior to the ophthalmic nerve (V1) and superior to the mandibular nerve (V3). From its root at the lateral aspect of the pons, V2 courses anteriorly toward the trigeminal cave and the middle cranial fossa, maintaining a strictly sensory profile compared with the mixed motor and sensory mandibular division. Neighboring cranial nerves are also represented as they emerge from the brainstem, providing immediate orientation to the pontine level of the cranial nerve nuclei and root entry and exit zones. Spatial relationships matter here. Small differences in superior-inferior position at the brainstem translate into distinct cisternal trajectories. This arrangement is clinically useful when localizing trigeminal neuropathies and differentiating lesions at the brainstem, trigeminal ganglion, or along the V2 course through foramen rotundum into the pterygopalatine fossa. V2 involvement produces sensory loss or dysesthesia over the midface, maxillary teeth, and palate, and it is often implicated in trigeminal neuralgia variants or perineural spread of maxillary sinus and palatal malignancies, where tumor tracks proximally toward the ganglion. For skull base surgery and endoscopic approaches to the pterygopalatine fossa, knowing V2’s relationship to V1 and V3 at their origin helps anticipate which division will declare itself first when symptoms cross territories.
