Description

Arising from the trigeminal nerve at the lateral aspect of the pons, the ophthalmic nerve (V1) occupies the most superior position of the three trigeminal divisions when the brain is viewed from beneath. From its root entry zone, V1 courses anteriorly toward the region of the cavernous sinus and superior orbital fissure, lying superior to the maxillary nerve (V2) and mandibular nerve (V3) along the anterolateral cranial base. Surrounding landmarks in this inferior perspective include the brainstem centrally, the cerebellum posteriorly (superior in the field), and the cerebral hemispheres framing the scene laterally with visible gyri and sulci. Cranial nerve structures radiate from the ventral brainstem, with the optic chiasm situated anterior to the hypothalamic region near the midline. Clean nerve topography. Clinically, accurate orientation of V1 from an inferior brain view supports teaching of cavernous sinus anatomy and the pattern of sensory loss across the ophthalmic dermatome. Lesions affecting V1 along its intracranial course, including meningioma at the anterior clinoid, cavernous sinus thrombosis, or herpes zoster ophthalmicus, can produce corneal hypesthesia and loss of the afferent limb of the corneal reflex (V1) with a preserved efferent blink (VII). This perspective also helps explain why a trigeminal schwannoma or surgical corridor near the petrous apex can shift multiple cranial nerves simultaneously, yet V1 remains the most superior trigeminal division relative to V2 and V3. Neuroanatomy and head and neck courses use this plate well when introducing cranial nerves in the basal view and when contrasting the trigeminal divisions before they enter the orbit, pterygopalatine fossa, or infratemporal fossa. It also fits neurosurgery and neuroradiology teaching files that correlate basal cistern relationships with clinical examination findings for trigeminal neuropathy.
