Description
Emerging from the pontomedullary junction, the facial nerve (cranial nerve VII) is seen from an inferior perspective with the intermediate nerve (nervus intermedius) running in close apposition along its medial aspect as the smaller sensory and parasympathetic root. Anterior and slightly medial to this complex lie the optic chiasm and adjacent basal forebrain structures, while posteriorly the cerebellar hemispheres frame the dorsal contour of the brainstem. Cranial nerve rootlets radiate from the ventral brainstem in orderly sequence, providing landmarks around the pons and medulla. Orientation is from the base of the brain. Nervus intermedius matters because it carries taste afferents from the anterior two thirds of the tongue via the chorda tympani and preganglionic parasympathetic fibers destined for the pterygopalatine and submandibular ganglia, functions that can be overlooked when VII is taught as a motor nerve to muscles of facial expression. This inferior angle mirrors the operative and radiologic logic used when localizing lesions at the cerebellopontine angle and internal acoustic meatus, where schwannomas and meningiomas may compress the VII-VIII complex and selectively affect lacrimation, salivation, or taste before obvious facial palsy appears. It also supports teaching about why Ramsay Hunt syndrome can combine facial weakness with otalgia and dysgeusia through involvement of the facial nerve root and its sensory component.
