Description
Arising at the pontomedullary junction, the intermediate nerve (nervus intermedius) is shown as the smaller, medial root that travels with cranial nerve VII, positioned between the motor root of the facial nerve and cranial nerve VIII (vestibulocochlear) at the cerebellopontine angle. From an inferior view of the male brain, the ventral surface of the pons and medulla oblongata sit centrally, with multiple cranial nerves radiating anteriorly and laterally from the brainstem. The optic chiasm lies anterior and superior to the brainstem on the basal diencephalon, with the optic tracts coursing posterolaterally toward the midbrain, while the cerebellum appears posterior to the brainstem and partly obscures dorsal landmarks such as the colliculi. Clinically, the nervus intermedius matters because it carries taste fibers from the anterior two thirds of the tongue (via the chorda tympani) and parasympathetic outflow to the lacrimal, submandibular, and sublingual glands, so lesions near the internal acoustic meatus can produce a mixed picture that pure facial motor palsy does not. This inferior brainstem perspective supports teaching the root entry and exit zones at the cerebellopontine angle, a region implicated in vestibular schwannoma mass effect and in surgical corridors for microvascular decompression and CPA tumor resection. Small nerve, big symptoms. Use this plate in neuroanatomy and cranial nerve modules to contrast the motor root of VII with the intermediate nerve and the adjacent VIII nerve, or in otology and skull base references discussing facial nerve dysfunction, dysgeusia, and lacrimation changes from proximal facial nerve involvement.
