Arising from the trigeminal nerve as V3, the mandibular nerve is highlighted from an inferior view as it exits the middle cranial fossa through the foramen ovale, positioned posterolateral to the foramen rotundum and anteromedial to the foramen spinosum. Nearby on the basal surface, the optic chiasm sits anterior and midline, while the brainstem occupies a central position with the pons superior to the medulla and the cerebellar hemispheres lying posterior. Yellow cranial nerves radiate from the brainstem and basal forebrain in predictable anterior to posterior order. Spatial relationships read cleanly from below. For teaching skull base anatomy, this angle matters because the foramen ovale is a hard bony waypoint for both diagnosis and intervention involving V3. Percutaneous procedures for trigeminal neuralgia that target the trigeminal ganglion often pass through the foramen ovale under fluoroscopic or CT guidance, and small deviations can injure adjacent structures such as the lesser petrosal nerve region, the middle meningeal artery territory near foramen spinosum, or venous channels around the foramen lacerum. It is a tight neighborhood. The inferior projection also reinforces which trigeminal division is purely sensory (V1, V2) versus mixed, since V3 carries motor fibers to the muscles of mastication.
