Description
Arising from the maxillary division (V2) of the trigeminal nerve, the zygomatic nerve enters the orbit through the inferior orbital fissure and divides into zygomaticofacial and zygomaticotemporal branches along the lateral orbital wall. The zygomaticotemporal nerve courses superolaterally, passing through the zygomatic bone via the zygomaticotemporal foramen to reach the temporal fossa, where it runs deep to the temporalis muscle and then ascends toward the anterior temporal scalp. Medially, the orbit and nasal cavity frame its proximal course, while laterally the zygomatic arch and temporal region define its exit point. Small communicating fibers toward the lacrimal nerve (V1) may be indicated near the lateral canthus. A small nerve with outsized teaching value. Anterior orientation makes the nerve’s orbital-to-temporal trajectory easy to read, which is exactly where learners often lose it in lateral skull views. Clinically, the zygomaticotemporal nerve matters during temporalis fascia elevation and zygomatic arch approaches, where traction or transection can produce postoperative numbness over the anterior temple and can contribute to painful neuroma. It is also part of the pathway carrying secretomotor fibers (via a communication to the lacrimal nerve) relevant to lacrimal gland innervation, a point that comes up in discussions of trigeminal and facial nerve cross talk.
