Description
Running vertically within the forehead soft tissues, the supraorbital nerve is traced from the orbit to the frontal scalp after exiting the superior orbital margin at the supraorbital notch or foramen. The nerve lies superior to the orbital septum and passes deep to the frontalis, distributing sensory branches across the superomedial forehead while staying lateral to the midline. Nearby landmarks include the supraorbital rim, upper eyelid tissues adjacent to orbicularis oculi, and the frontal belly of occipitofrontalis; the accompanying supraorbital vessels typically course in close proximity as they ascend from the orbit. Clinical work around the brow begins with this anatomy. Blocking the supraorbital nerve, a terminal branch of the ophthalmic division (V1) of the trigeminal nerve, is a standard technique for laceration repair, brow surgery, and perioperative analgesia, and the notch or foramen at the superior orbital margin is the palpation target for accurate anesthetic placement. Entrapment or irritation at that bony exit point can contribute to supraorbital neuralgia and may also be implicated in frontal headache syndromes, while iatrogenic neuropraxia remains a recognized risk during endoscopic brow lift and forehead flap elevation.
