Description
Running obliquely along the anterolateral cervical region, the sternocleidomastoideus of an adult male extends superiorly to the mastoid process and lateral part of the superior nuchal line, then descends to its dual inferior attachments on the manubrium sterni (sternal head) and medial clavicle (clavicular head). The two heads frame a small supraclavicular fossa inferiorly, while the muscle belly forms a prominent superficial landmark that separates the anterior cervical triangle from the posterior triangle. Deep to its midportion you would expect the carotid sheath contents to lie medial, and the underlying scalene compartment and brachial plexus to lie more posterior and inferior. Surface anatomy hinges on this muscle. Palpation and line of sight from the mastoid to the sternoclavicular region help you localize the carotid pulse, estimate the level of the carotid bifurcation, and orient neck incisions in open vascular and endocrine surgery. Clinically, hypertrophy or fibrotic shortening of the sternocleidomastoideus underlies congenital muscular torticollis, where the head tilts toward the affected side and rotates away, and its superficial course places the external jugular vein at risk during trauma or central line attempts at the root of the neck. A reliable landmark.
