Description
Posteriorly oriented through a transparent trunk, the abdominal esophagus descends through the esophageal hiatus of the diaphragm to join the stomach at the cardia, with the fundus positioned superior and left of the gastroesophageal junction and the body sweeping inferomedially toward the antrum. Along the medial border, the lesser curvature tracks toward the pylorus, while the greater curvature bulges laterally and inferiorly, draping toward the transverse colon. Rugae line the gastric lumen as prominent mucosal folds against the muscular wall. Distal to the pyloric canal, the duodenum begins as the proximal small intestine, while jejunoileal loops occupy the central and lower abdomen, framed peripherally by the ascending, transverse, descending, and sigmoid colon. A posterior view clarifies how the stomach sits deep to the left hemidiaphragm and relates to adjacent viscera, a relationship that matters when localizing pain and interpreting cross sectional imaging. The gastroesophageal junction and proximal stomach are common sites of pathology in older adults, including reflux related esophagitis, Barrett metaplasia, and adenocarcinoma, and this orientation helps when teaching how a hiatal hernia can displace the cardia and fundus superiorly into the thorax. Surgical landmarks also read cleanly in this projection, from the pylorus and first part of the duodenum to the surrounding colonic frame, which influences trocar placement and dissection planes in partial gastrectomy or pyloroplasty.
