Description
Arising from the styloid process of the temporal bone, the stylomandibular ligament descends anteroinferiorly as a thickened band of deep cervical fascia to reach the posterior border of the mandibular ramus near the angle of the mandible. Lateral skull and upper neck anatomy frames its course, with the parotid region immediately lateral and the pharyngeal wall and stylohyoid complex positioned more medially. The mandible, zygomatic bone, and adjacent masticatory musculature set the bony and muscular landmarks around its distal insertion. Fine cranial detail, including the external acoustic region and middle ear ossicles, orients the viewer to the temporal bone source of the ligament. Functionally, the stylomandibular ligament acts as a passive checkrein that limits excessive mandibular protrusion and helps guide terminal opening of the jaw, so this lateral orientation is the clearest way to appreciate how a short, firm fascial condensation can influence mandibular kinematics at the temporomandibular joint. Clinically, it becomes relevant when evaluating trismus or restricted mouth opening, where pain or fibrosis in the parotid and peristyloid region can be mistaken for primary TMJ disc pathology, and when planning surgical corridors near the mandibular angle and styloid process.
