Description
Spanning the root of the neck into the axilla, the subclavian artery is presented in relation to the clavicle and the anterior scalene muscle, allowing its classic three-part division to be read proximodistally as it arches laterally toward the upper limb. Medial to the anterior scalene, the first part lies deep to the sternocleidomastoid and infrahyoid strap muscles and adjacent to the brachial plexus roots, while the artery’s second part courses posterior to the anterior scalene within the interscalene interval. Lateral to the anterior scalene, the third part tracks toward the lateral border of the first rib where it continues as the axillary artery, with the subclavian vein positioned more anterior and inferior, separated from the artery by the scalene musculature. Landmarks are clear. Understanding this morphology matters because the scalene muscles define both surgical corridors and common compression sites. Catheter-based access and open exposure at the thoracic outlet rely on knowing where the vessel sits relative to the clavicle and first rib, and where the phrenic nerve runs on the anterior scalene while the brachial plexus trunks sit superior and posterior to the artery. This relationship also frames clinical discussions of thoracic outlet syndrome, post-traumatic clavicular and first-rib injuries, and the risk of iatrogenic vascular injury during central venous cannulation or supraclavicular approaches.
