Description
Emerging from the lateral cord of the brachial plexus in the axilla, the musculocutaneous nerve courses inferiorly and laterally into the anterior compartment of the arm, closely related to the humerus and then the radius and ulna of the forearm. After supplying the biceps brachii and brachialis, it continues distal to the elbow as the lateral antebrachial cutaneous nerve, running along the lateral (radial) side of the forearm toward the wrist and hand. The anterior perspective also places the clavicle, ribs, and proximal thorax superiorly, with the nerve pathways clearly contrasted against the skeletal landmarks. Distal branches can be appreciated against the carpals, metacarpals, and phalanges. Clinically, this view clarifies why isolated musculocutaneous injury can present with weak elbow flexion and reduced sensation over the lateral forearm while sparing the median nerve mediated thenar function and most palmar sensation. It also helps differentiate lesions at the level of the lateral cord versus more distal entrapment within or between the coracobrachialis and biceps brachii, a pattern sometimes seen after shoulder surgery, traction injury, or anterior shoulder dislocation. A clean sensory endpoint. The continuity into the lateral antebrachial cutaneous nerve is a frequent exam point and a practical landmark during venipuncture or surgical approaches near the cephalic vein and lateral elbow.
