Description

Centered in the anterior mediastinal field, the heart sits with the right atrium and right ventricle more anterior than the left-sided chambers, while fine cardiac nerves trace toward the coronary grooves and the bases of the great vessels. Superiorly, the ascending aorta continues into the aortic arch, with the pulmonary trunk positioned anterior and left of the aorta before bifurcating into the pulmonary arteries. Delicate neural filaments and small ganglionic elements cluster around the roots of the aorta and pulmonary trunk, consistent with the superficial and deep cardiac plexuses, while the trachea lies superior and posterior to the great arteries. Green lymphatic channels and nodes course along the tracheobronchial region and the pericardial reflections. Color coding separates arteries (red), veins (blue), and lymphatics (green) for rapid orientation. Anterior cardiac innervation matters because the autonomic fibers reach the heart by traveling with the great arteries and then distributing along the coronary vasculature, a pathway that explains why surgical work at the aortic root, pulmonary artery, or pericardial reflections can provoke dysrhythmia. Parasympathetic input via the vagus nerve and sympathetic input via cervical and upper thoracic cardiac nerves converge in the cardiac plexus, then modulate sinoatrial and atrioventricular nodal conduction and coronary tone.
