Description
Arising from the infraorbital nerve as it exits the infraorbital foramen, the anterior and middle superior alveolar nerves descend within the anterior wall of the maxilla to form the superior dental plexus over the roots of the maxillary dentition. Incisors and canines sit anterior and inferior to the infraorbital foramen, while premolars and molars occupy a more posterior position along the maxillary arch, with their apices projecting superiorly toward the floor of the maxillary sinus. A translucent maxilla allows the course of these neural branches to be followed through the alveolar process toward the gingiva and periodontal tissues. Clean anatomy. For dental anesthesia, this relationship matters. Local infiltration over the canine fossa targets anterior superior alveolar branches for maxillary incisors and canines, while an infraorbital nerve block at the infraorbital foramen can capture the anterior superior alveolar supply and adjacent facial sensation, but variable contribution from the middle superior alveolar nerve often explains incomplete premolar anesthesia. Proximity of posterior root apices to the maxillary sinus also frames odontogenic sinusitis and referred pain patterns in the midface.
