Description
Running obliquely across the anterolateral neck, the omohyoid appears as a paired infrahyoid strap muscle with an inferior belly arising from the superior border of the scapula near the suprascapular notch, an intermediate tendon tethered to the clavicle and deep cervical fascia, and a superior belly ascending to the body of the hyoid. Medially it lies deep to platysma and superficial cervical fascia, then passes deep to the sternocleidomastoid as it crosses toward the midline, with the superior belly bordering the lateral aspect of the sternohyoid. The intermediate tendon typically overlies elements of the carotid sheath region, with the internal jugular vein and common carotid artery positioned deep and slightly posterior. Small fascial planes and adjacent strap muscles frame it cleanly. Surgeons use the omohyoid as a working landmark in selective neck dissection because it helps define cervical lymph node levels III and IV and signals where the internal jugular vein is likely to be encountered deep to the sternocleidomastoid. Its fascial sling can also matter in venous return, as tightening of the intermediate tendon during head rotation has been implicated in omohyoid syndrome with intermittent neck swelling, and the muscle is commonly divided or retracted during anterior approaches for carotid endarterectomy and central venous access.
