Description
Palmar-side anatomy of the ulnar wrist is presented with the fingers directed inferiorly and the distal forearm superiorly, allowing the palmar ulnocarpal ligament complex to be traced from the ulna toward the proximal carpal row. The ulnocarpal fibers span from the ulnar styloid region and adjacent palmar radioulnar ligament area into the triquetrum and lunate, blending medially with the ulnar collateral ligament and distally with the palmar carpal ligaments. Surrounding osseous landmarks include the metacarpals and phalanges distally, and the paired carpal rows proximally. The ligament bands sit anterior to the carpal bones and articulate surfaces, defining the volar contour of the ulnocarpal articulation. Clear. Direct. This anterior perspective matters when teaching and treating ulnar-sided wrist pain, where the palmar ulnocarpal ligaments and the triangular fibrocartilage complex (TFCC) share attachments and load transmission pathways between the distal ulna and the carpus. Traumatic hyperextension with ulnar deviation, or degenerative TFCC thinning, can stress the ulnotriquetral and ulnolunate components and contribute to distal radioulnar joint (DRUJ) instability and ulnocarpal impaction patterns. For operative planning, the palmar view aligns with volar approaches and helps clarify why repair or reconstruction must respect the relationship of these fibers to the ulnar styloid foveal region and the proximal carpal row.
