Description
Running along the anterior aspect of the knee, the patellar ligament extends from the apex and adjacent margins of the patella inferiorly to the tibial tuberosity on the proximal tibia. From a medial view, the patella sits anterior to the distal femur, while the medial tibial plateau lies inferior and medial, capped by articular cartilage at the femorotibial contact surface. Superiorly, the quadriceps tendon continues onto the patella and then transitions seamlessly into the patellar ligament as the extensor mechanism crosses the joint. Clear bony landmarks anchor the orientation. Medial profiling of the patellar ligament matters because it clarifies how tensile forces from quadriceps contraction are transmitted across the patellofemoral joint and concentrated at the tibial tuberosity, a relationship that underlies patellar tendinopathy (jumper’s knee) and traction apophysitis in younger patients. Orthopedic approaches that involve a medial parapatellar arthrotomy, as in total knee arthroplasty, use this anatomy as a guide while protecting the integrity of the extensor mechanism. Alignment errors here are costly. Patella alta or baja can also be discussed in this context, since altered patellar height changes ligament length and patellofemoral contact mechanics. Educators can drop this plate directly into lower limb anatomy modules, kinesiology teaching on the knee extensor apparatus, or surgical training materials that introduce the medial approach to the knee.
