Description
Viewed from the ventral (basal) aspect, the pia mater is rendered as a delicate meningeal membrane closely investing the inferior surfaces of the cerebral hemispheres, draping into the sulci while remaining adherent to the gyri. Along the midline, pia follows the contours of the brainstem from the medulla through the pons to the midbrain, with the cerebellar hemispheres and vermis positioned posteroinferior to the cerebrum. Cranial nerve rootlets emerge through the subarachnoid space and appear in close apposition to the pial surface, most conspicuously the optic nerves (CN II) converging anteromedially to form the optic chiasm, highlighted in yellow against the basal forebrain. That basal pial relationship matters when you are teaching why subarachnoid hemorrhage tracks along cisterns and around cranial nerves rather than dissecting within brain parenchyma. The optic chiasm region is a key landmark for correlating compressive patterns from pituitary macroadenoma or suprasellar aneurysm with visual field loss, and the pial investment helps clarify why hemorrhage or inflammation can irritate CN II, III, and adjacent perforators within the chiasmatic and interpeduncular cisterns.
