Description

Arising from the basilar artery along the ventral (anterior) surface of the pons, the pontine arteries appear as numerous short perforators coursing posteriorly into pontine parenchyma, arranged on either side of the basilar midline sulcus. An inferior perspective brings the vertebrobasilar junction, proximal basilar trunk, and the lateral pontine region into the same field, where circumferential branches track laterally toward the cerebellopontine angles. Cranial nerves emerging from the brainstem are positioned in close proximity, with the abducens nerve (CN VI) classically related to the pontomedullary junction and the facial and vestibulocochlear nerves (CN VII and CN VIII) more lateral at the pontomedullary sulcus. Small vessels dominate. Their density is the point. Pontine perforators matter because they behave like end arteries, and their occlusion produces focal brainstem syndromes with tightly localizing exam findings. Lipohyalinosis from long-standing hypertension commonly affects these small penetrating branches, leading to lacunar infarcts in the basis pontis and acute deficits such as dysarthria-clumsy hand syndrome or pure motor hemiparesis depending on corticospinal tract involvement. This view also supports surgical and endovascular planning around the basilar trunk, where even minor vessel compromise during basilar aneurysm treatment or skull base exposure can translate into disproportionate morbidity. Neuroanatomy and neurovascular teaching benefit from this layout when you need to correlate the basilar artery and pontine branches with cranial nerve root exit zones and brainstem surface landmarks.
