Description
Arising from the venous plexus around the pancreatic head and second part of the duodenum, the posterior superior pancreaticoduodenal vein courses on the posterior surface of the duodenum, closely applied to the pancreatic parenchyma and deep to the gastroduodenal region. From a posterior aspect in the male abdomen, it is seen converging with adjacent pancreaticoduodenal tributaries and draining superiorly toward the portal venous system at the level of the neck of the pancreas. Nearby landmarks in this projection include the portal vein posterior to the pancreas, the abdominal aorta in the retroperitoneum, and the liver superiorly with the stomach and spleen to the left. For teaching, this view gives the clearest sense of how posterior pancreaticoduodenal venous channels sit against the retroperitoneum while remaining tethered to the head of pancreas. That relationship matters in pancreaticoduodenectomy (Whipple) and in control of venous bleeding along the posterior pancreatic head, where small pancreaticoduodenal veins can avulse during kocherization of the duodenum or dissection near the portal vein and superior mesenteric vein confluence. Portal hypertension also recruits and distends pancreaticoduodenal collateral pathways, a relevant detail when interpreting variceal patterns around the duodenum or pancreatic head on cross sectional imaging. Small vessel, big consequence.
