Arising along the posterior aspect of the pancreatic head, the posterior superior pancreaticoduodenal vein courses in the pancreaticoduodenal groove, hugging the medial, concave border of the second part of the duodenum as it ascends toward the hepatoduodenal region. Proximally it collects venules from the posterior duodenal wall and adjacent pancreatic parenchyma, then passes superiorly and medially to join the peripancreatic venous arcade near the portal venous confluence. Surrounding context includes the stomach and pancreas inferior to the liver, with the thoracic organs superiorly and the abdominal arterial and venous trunks providing orientation. Venous anatomy in this region matters because the pancreaticoduodenal veins form a key porto-systemic interface and a frequent route for collateral flow when portal venous drainage is impaired. In pancreatitis or pancreatic head carcinoma, edema and mass effect can narrow the peripancreatic venous channels, complicating venous return and increasing intraoperative bleeding risk during a Kocher maneuver or pancreaticoduodenectomy.
