Description
Centered on the pancreatic head and neck, the anterior superior pancreaticoduodenal vein courses along the anterior aspect of the duodenum and pancreaticoduodenal groove, collecting venous tributaries (venules) from the proximal duodenum and adjacent pancreatic parenchyma. Medially, its drainage converges toward the portal venous inflow region behind the pancreatic neck, where the superior mesenteric vein and splenic vein form the portal vein. Laterally and superiorly, the splenic vein tracks along the posterior-superior border of the pancreatic body toward the spleen, which sits leftward in anatomical position but may appear on the viewer’s right depending on layout. Small peripancreatic vessels frame the C loop of the duodenum. Anterior venous anatomy around the pancreatic head matters when you are teaching or planning approaches to pancreaticoduodenectomy (Whipple procedure), where venous bleeding often comes from short, thin-walled pancreaticoduodenal tributaries during kocherization and uncinate dissection. That is where trouble starts. This view also supports interpretation of pancreatic head masses and pancreatitis, because inflammatory edema or tumor can distort the pancreatic neck region and secondarily affect portal flow, contributing to segmental portal hypertension or the formation of peripancreatic varices.
