Description
Running across the head of the pancreas and the adjacent C loop of the duodenum, the anterior inferior pancreaticoduodenal vein tracks in close company with the anterior inferior pancreaticoduodenal artery, forming a paired vessel course on the anterior aspect of the pancreaticoduodenal region. The pancreatic body extends leftward to a tapering tail that approaches the splenic hilum, with the spleen positioned lateral to the pancreatic tail. Anterior orientation clarifies how the venous channel lies superficial to deeper retroperitoneal planes while remaining tethered to the pancreatic head and duodenal wall. Small tributaries from the duodenum and pancreatic parenchyma converge into this vein along the inferior margin of the pancreatic head. Surgeons and interventionalists care about this territory because the pancreaticoduodenal arcades provide key collateral pathways between the celiac axis and the superior mesenteric artery, and the accompanying veins can be a source of brisk bleeding when the plane is entered during a pancreaticoduodenectomy (Whipple procedure) or when mobilizing the duodenum in a Kocher maneuver. Venous anatomy here is also clinically relevant in pancreatitis and groove pancreatitis, where inflammatory edema and enzymatic injury can obscure the vessel wall and increase the risk of hemorrhage or pseudoaneurysm-related complications in the neighboring arterial arcade. Good landmarks matter. Use this illustration in gross anatomy and GI surgery teaching to orient learners to the anterior relationships of the pancreatic head, duodenum, and splenic tail, and in operative atlases or review articles discussing pancreaticoduodenal vascular control, lymphadenectomy planes, and sources of intraoperative hemorrhage.
