Description
Arising from the superior mesenteric artery, the posterior inferior pancreaticoduodenal artery courses superiorly and medially behind the head of the pancreas, hugging the posterior aspect of the duodenal C loop. From a posterior perspective, its arc is seen deep to the pancreatic parenchyma where it contributes to the pancreaticoduodenal arcade, with small branches passing anteriorly into the pancreatic head and posteriorly toward the second and third parts of the duodenum. The splenic hilum and spleen sit leftward, with the pancreatic tail extending laterally toward them while the gland’s neck and head remain anchored to the right, close to the retroperitoneum. Short, caliber-tapering arterioles within the gland emphasize the dense vasculature that supports both exocrine acini and endocrine islets. Posterior pancreaticoduodenal vessels matter because they sit in the operative plane for pancreatic head surgery and for mobilization of the duodenum (Kocher maneuver), where traction and cautery can injure the arcade and produce difficult-to-control retroperitoneal bleeding. The inferior pancreaticoduodenal system also forms a key collateral pathway between the superior mesenteric artery and celiac circulation, so celiac trunk stenosis can drive enlarged flow through the pancreaticoduodenal arcade and predispose to true aneurysms, a well-described source of upper gastrointestinal hemorrhage when ruptured. Small structure, big consequences. Educators can drop this plate into gross anatomy and GI block teaching to clarify how the pancreas, duodenum, and splenic relationship frame the arterial supply, and why posterior vessels are harder to appreciate in standard anterior views.
