Description
Oriented from a posterior perspective, the posterior superior pancreaticoduodenal vein is traced along the posterior aspect of the pancreatic head as it courses adjacent to the C-shaped duodenum. Proximally, the vessel lies in close relationship to the common bile duct as the duct descends toward the second part of the duodenum, with the pancreas positioned medial to the duodenal loop and the spleen extending laterally toward the tail of the pancreas. Venous channels are emphasized against the gland’s lobulated surface, clarifying the short, converging tributaries that drain the pancreaticoduodenal region. Small vessel, tight neighborhood. Posterior venous anatomy around the pancreatic head matters because it sits directly in the operative corridor for pancreaticoduodenectomy and for mobilization of the duodenum during a Kocher maneuver. Bleeding from pancreaticoduodenal veins is a common source of obscured planes during dissection, and the posterior superior pancreaticoduodenal vein is one of the small-caliber connections that can tether the head of the pancreas to the retropancreatic venous confluence. This view also supports teaching of how venous drainage from the pancreatic head relates to the biliary tree, a relationship that becomes clinically relevant in pancreatic head carcinoma and inflammatory mass lesions that distort the common bile duct.
