Description
Arising from the superior mesenteric artery in the infrapancreatic region, the anterior inferior pancreaticoduodenal artery courses superiorly and to the right along the anterior surface of the pancreatic head, hugging its inferior margin before turning toward the C loop of the duodenum. Branches track laterally within the pancreaticoduodenal groove, running anterior to the distal common bile duct and proximal to the horizontal (third) part of the duodenum. The pancreatic body extends leftward toward the splenic hilum, placing the artery’s territory in clear contrast to the splenic artery distribution along the superior pancreatic border. Orientation is anterior, with vascular caliber and branching emphasized against the lobulated pancreatic parenchyma. Clear landmarks. For pancreaticoduodenal blood supply, this vessel matters because it forms the inferior component of the pancreaticoduodenal arcade, an anastomotic circuit with the superior pancreaticoduodenal arteries (from the gastroduodenal artery) that links the SMA and celiac trunk territories. In pancreatic head resections (Whipple procedure) and duodenal mobilization (Kocher maneuver), surgeons encounter and ligate branches in this region, and unrecognized variation can drive troublesome bleeding along the anterior pancreaticoduodenal groove. The same arcade becomes a collateral pathway in celiac axis stenosis or occlusion, where increased flow may predispose to pancreaticoduodenal artery aneurysm, a diagnosis often established on CT angiography before endovascular embolization or surgical repair. Ideal applications include gross anatomy teaching on foregut to midgut transition, surgical anatomy figures for hepatopancreatobiliary operative atlases, and radiology correlation plates for arterial phase CT or catheter angiography of the SMA.
