Retinal microanatomy is organized in laminated strata from the retinal pigment epithelium (RPE) externally to the nerve fiber layer internally, with photoreceptor outer segments apposed to the pigmented epithelium and separated from the choroid by Bruch’s membrane at the outer border. Rod and cone cell bodies occupy the outer nuclear layer, synapsing in the outer plexiform layer with bipolar and horizontal cells whose somata lie in the inner nuclear layer. More internally, the inner plexiform layer contains bipolar, amacrine, and ganglion cell synapses, while ganglion cell bodies form the ganglion cell layer and their axons converge within the nerve fiber layer along the vitreal surface, deep to the internal limiting membrane. Layer specificity matters in ophthalmology because many disorders respect these boundaries. Rhegmatogenous retinal detachment develops at the interface between neurosensory retina and RPE, a surgical and diagnostic distinction that explains subretinal fluid collections and photoreceptor compromise. Glaucoma and other optic neuropathies selectively thin the retinal nerve fiber layer and ganglion cell complex, while diabetic retinopathy and retinal vein occlusion preferentially disrupt the inner retinal circulation, producing edema that tracks through the inner nuclear and plexiform layers. Correlation with optical coherence tomography is straightforward here, since the alternating nuclear and plexiform bands mirror the reflectivity patterns clinicians measure every day. Neuroanatomy and histology courses use this illustration to teach how a “simple” sensory epithelium implements pre-cortical processing through horizontal and amacrine circuitry, and why the retina is often treated as accessible central nervous system tissue.
