Emerging from the optic chiasm, the optic nerve (cranial nerve II) courses anterolaterally toward the orbital apex, where it passes through the optic canal of the sphenoid to enter the orbit and continue to the posterior pole of the globe at the optic disc. Medial to the nerve in the canal and at the apex runs the ophthalmic artery, a branch of the internal carotid artery, following it into the orbit in close apposition before distributing branches to the retina and orbital contents. Proximally, the optic tract lies posterolateral to the chiasm on the ventral diencephalon as it heads toward the lateral geniculate body. Tight quarters. That nerve artery relationship is the teaching point. It explains why lesions at the optic canal and orbital apex can combine visual loss with ocular ischemic findings, and why compressive optic neuropathy from optic nerve sheath meningioma or sphenoidal pathology may coexist with altered retinal perfusion through ophthalmic artery compromise. For surgeons, the corridor matters: endoscopic endonasal decompression of the optic canal, anterior clinoidectomy, or management of carotid ophthalmic aneurysms all demand an exact mental map of how CN II, the ophthalmic artery, and the bony canal stack and cross. Neuroanatomy and ophthalmology courses use this kind of rendering to connect retina, optic nerve, chiasm, tract, and central visual pathways without losing the key vascular neighbor that accompanies CN II into the orbit. It also fits well in figure panels for neurosurgical atlases, ophthalmic plastics references on orbital apex syndrome, and patient education on traumatic optic neuropathy in adult male head injury.
