Description
Arising from the optic chiasm on the ventral surface of the diencephalon, the paired optic nerves (CN II) run anterolaterally toward the orbital apices, with the chiasm positioned anterior to the infundibulum and superior to the diaphragma sellae region. From this inferior perspective, the optic tracts continue posteriorly from the chiasm along the cerebral peduncles toward the lateral geniculate bodies, bordering the hypothalamic area medially. Nearby cranial nerves emerge from the brainstem, with the midbrain, pons, and medulla arranged along the midline, and the cerebellum lying posterior and inferior to the brainstem. Orientation of CN II at the skull base matters because lesions at the chiasm and proximal tracts produce pattern-specific field deficits that localize pathology before imaging does. A pituitary macroadenoma extending superiorly can compress the central chiasm from below, classically causing bitemporal hemianopia, while a tuberculum sellae meningioma more often impinges anteriorly and can present with unilateral optic neuropathy. This angle also supports surgical planning for endoscopic endonasal and transcranial approaches, where the relationship between the chiasm, optic nerves, and infundibulum determines working corridors and risk to perforators. Localization is the point.
