Description
Posteriorly oriented anatomy frames the gallbladder’s serosa (tunica serosa) as a thin, glistening peritoneal coat draped over the fundus, body, and tapering neck. Superior to it lies the visceral surface of the liver, and along the hepatic (superior) surface the serosa blends into the subserosal connective tissue where the gallbladder sits in its fossa, leaving an area that is peritoneal only at the margins. Medially and inferiorly, the contour leads toward the region where the cystic duct exits the neck to join the biliary tree, with the cholecystic (biliary) relationships implied just deep to this outer layer. A posterior angle is practical because it approximates the operative view as the gallbladder is retracted and rotated off the liver bed during laparoscopic cholecystectomy, exposing peritoneal reflections and the plane of dissection. That plane matters when acute cholecystitis thickens the serosa and subserosa, obscures normal areolar tissue, and makes bleeding from the liver bed more likely as the surgeon strays deep. Serosal integrity also underpins staging in suspected gallbladder carcinoma, where invasion beyond the muscularis into perimuscular connective tissue and serosa shifts prognosis and surgical planning.
