Description
Superficial skin and subcutaneous fat are reflected to expose the posterolateral thigh, where the short head of the biceps femoris lies deep to the fascia lata and lateral to semitendinosus and semimembranosus. Its muscle belly arises from the lateral lip of the linea aspera and adjacent lateral supracondylar line of the femur, then descends inferolaterally to converge with the long head toward the fibular (peroneal) head. Proximally it sits anterior and slightly medial to the long head, then becomes more posterior as the two components form a common tendon at the lateral margin of the popliteal fossa. A clear lateral boundary. That relationship matters when you teach, image, or operate around the posterolateral knee: the biceps femoris tendon forms a key landmark for the common fibular nerve as it tracks along the medial border of the biceps tendon before winding around the fibular neck. Strains at the myotendinous junction and tendinopathy near the fibular insertion are common in sprinting and kicking athletes, and the short head can be selectively involved because it is innervated by the common fibular division of the sciatic nerve, unlike the tibial-innervated long head. Posterolateral corner reconstructions, hamstring harvest planning, and exam maneuvers for lateral knee pain all benefit from seeing the short head in situ under superficial tissues.
