Description
Running along the posterolateral thigh, the long head of the biceps femoris forms a broad fusiform belly that becomes superficial beneath the skin as it descends toward the knee. Medially, the hamstring mass is completed by semitendinosus and semimembranosus, while laterally the long head blends proximally with the posterior compartment contours and distally converges toward the fibular head. Near the distal third of the thigh, the biceps femoris tendon defines the lateral boundary of the popliteal fossa as it passes posterolateral to the knee. Superficial fascial planes and dermal contouring emphasize how close this muscle lies to the surface in the male posterior view. Clear landmarks. Posterior depiction of the long head matters because most clinically relevant hamstring injuries occur near the myotendinous junction and proximal free tendon, and biceps femoris strains often present as posterolateral thigh pain with weakness in knee flexion and impaired hip extension. The distal attachment to the head of the fibula also puts the biceps femoris in the same surgical neighborhood as the common fibular (peroneal) nerve as it courses around the fibular neck, a relationship that explains neuropraxia risk during lateral knee trauma or posterolateral corner procedures. For teaching, the lateral hamstring is also the easiest way to orient students to the boundaries of the popliteal fossa and to distinguish hamstrings from the gastrocnemius heads inferiorly.
