Description
Crista galli rises from the midline of the ethmoid bone as a sharp, vertical crest immediately superior to the cribriform plate. In this fixed view of the anterior cranial base, the crista galli sits between the paired cribriform plates, with their perforations for olfactory nerve filaments arranged on either side of the midline. Inferiorly, the crest is continuous with the ethmoidal labyrinth and the perpendicular plate region, while superiorly it projects toward the frontal bone and the anterior cranial fossa floor. Falx cerebri attachment is implied at the superior margin. Small but defining. Clinical work around the crista galli often starts with its neighbors: the cribriform plate and the frontoethmoidal junction, where trauma can produce cerebrospinal fluid rhinorrhea and anosmia after anterior skull base fractures. Otolaryngologists and skull base surgeons also use the crista galli as a midline landmark during endoscopic approaches to the cribriform region, planum sphenoidale, and olfactory groove, where orientation errors risk breaching the cribriform plate. The bony prominence also matters in neuroradiology and operative planning for falcine and olfactory groove meningiomas, since the falx anchors along the crista galli and adjacent dura.
