Description
Running through the infratemporal region on a sagittal section, the sphenomandibular ligament appears as a flattened fibrous band descending from the spine of the sphenoid toward the lingula on the medial surface of the mandibular ramus, just superior to the mandibular foramen. Medial pterygoid fibers sit anteromedial to the mandibular angle, while the maxilla and dentition define the anterior facial skeleton in profile. Posteriorly, the temporal bone and mastoid process anchor the cranial base, and inferiorly the hyoid and upper cervical vertebrae align beneath the skull with the normal cervical lordosis and intervening intervertebral discs. Bone landmarks read cleanly. Anatomically, this is the ligament classically described as a remnant of Meckel cartilage, and its relationship to the lingula matters when you are teaching or planning access to the inferior alveolar neurovascular bundle. The sphenomandibular ligament can act as a tactile and spatial reference near the mandibular foramen, a region central to inferior alveolar nerve block technique and to medial ramus osteotomy work, even though the ligament itself is not the primary restraint of the temporomandibular joint. Its proximity to the infratemporal fossa also helps clarify why needle trajectory and depth errors can land in medial pterygoid, producing trismus.
