Description
Running obliquely across the anterolateral neck, the sternocleidomastoideus is isolated in lateral profile from its superior attachment at the mastoid process of the temporal bone and the lateral portion of the superior nuchal line to its inferior attachments on the manubrium sterni and medial clavicle. Posterior to its belly, the cervical vertebrae form the bony column of the neck, while the clavicle lies inferior and anterior to the scapula at the shoulder girdle. The mandible and maxilla with dentition, the orbit, and the nasal cavity orient the muscle’s line of pull relative to the craniofacial skeleton. Clear landmarks. Functionally, this view is the one clinicians think in when palpating the sternocleidomastoid for spasm in acute torticollis or when assessing head posture and rotation deficits after whiplash. Surgeons also use the muscle as a surface landmark: the carotid sheath and carotid triangle lie deep to its anterior border, and the spinal accessory nerve (CN XI) traverses the posterior triangle near its posterior margin, a location at risk during cervical lymph node biopsy or neck dissection. The muscle’s dual sternal and clavicular heads also help explain the paired tendinous bands you can feel just superior to the sternoclavicular joint.
