Description
Seen from an inferior perspective, the submental lymph nodes and their afferent and efferent lymphatic vessels cluster in the midline of the suprahyoid neck, immediately inferior to the mandibular symphysis and anterior to the hyoid region. The lymphatics lie superficial to the mylohyoid and anterior bellies of the digastric, bordered laterally by the submandibular spaces and coursing posteriorly toward the submandibular lymph nodes along the inferior border of the mandible. Surrounding anatomy logically includes the platysma and anterior neck fascia superficially, with the sternocleidomastoid more posterolateral and the floor of mouth musculature deep to the nodal group. Tight spacing. Clear midline relationships. This view matters because submental drainage is a predictable pathway for malignancy and infection from the central lower lip, anterior floor of mouth, mandibular incisors, and chin skin, with first-echelon spread often reaching the submental nodes before the submandibular chain. Surgeons planning selective neck dissection levels Ia and Ib, or clinicians staging oral cavity squamous cell carcinoma, use these landmarks to distinguish midline suprahyoid nodal disease from more lateral submandibular involvement. It also helps explain why odontogenic infections can present as submental swelling and, when they track through fascial planes, threaten airway space in the suprahyoid neck.
