Running superficially across the posterior neck, the superficial cervical vein is traced as it courses over the investing fascia and drains the subcutaneous tissues overlying the trapezius and adjacent posterior cervical musculature. Broad trapezius bellies occupy the midline from the external occipital region inferiorly toward the upper thoracic spine, while deeper muscle contours consistent with splenius capitis and semispinalis capitis appear medial to the posterior border of the sternocleidomastoid region. Laterally, the shoulders frame the field, with the deltoid contours and the superior aspects of the scapulae providing bony and muscular landmarks for orientation. Arteries appear in red and accompany the venous pathways where they pass between muscular planes. Posterior venous anatomy in the neck gets ignored until it complicates exposure. A superficial cervical vein can be a persistent source of bleeding during posterior cervical approaches, trapezius-splitting incisions, and subcutaneous dissection for occipital nerve stimulation leads or other posterior neck instrumentation. Its relationships to the trapezius fascia and to the muscular intervals between trapezius, splenius capitis, and semispinalis capitis also make it a useful landmark when teaching how the superficial venous network communicates with deeper venous channels near the nuchal region.
