Description
Running along the depth of the lateral sulcus (Sylvian fissure), the superficial middle cerebral vein tracks posteriorly over the lateral surface of the cerebral hemisphere, collecting venous tributaries from the frontal, parietal, and superior temporal opercula. Superior to it lie the inferior frontal and precentral gyri; inferiorly, the superior temporal gyrus borders the fissure, while the insula sits deep and medial to the opercular lips. Posteriorly, the vessel approaches the region of the supramarginal gyrus near the termination of the lateral sulcus. Color coding typically separates the superficial venous channel from adjacent cortical arteries and from the yellow cranial nerves at the skull base. A long venous corridor. A lateral presentation of this vein matters because it is the dominant superficial drainage pathway for much of the Sylvian region and frequently anastomoses with the cavernous or sphenoparietal venous routes and with the superior sagittal system via the vein of Trolard. During frontotemporal (pterional) craniotomy and Sylvian fissure splitting, surgeons often preserve the superficial middle cerebral vein and its bridging veins to avoid postoperative venous infarction of the temporal operculum or frontal operculum. The same corridor also frames common vascular pathology, including middle cerebral artery aneurysms and arteriovenous malformations, where venous outflow patterns guide operative and endovascular planning.
