Description
Arising from the common fibular (peroneal) nerve and carrying fibers from the lumbosacral plexus, the superficial fibular nerve courses distally within the lateral compartment of the leg, running deep to and then between fibularis longus and fibularis brevis. From a lateral perspective at the distal third of the shin, it becomes subcutaneous as it pierces the deep fascia, then divides into dorsal cutaneous branches that pass anterior to the lateral malleolus to supply the dorsum of the foot. The nerve tracks anterolateral to the fibula, while the calcaneus, tarsals, metatarsals, and phalanges provide the bony scaffold beneath the extensor tendons and retinacula. This lateral view matters because the superficial fibular nerve is exposed where it exits the crural fascia, a common site of traction or entrapment injuries and an anatomic hazard during anterolateral ankle arthroscopy portal placement. Distal branches are frequently encountered during open reduction and internal fixation of lateral malleolar fractures and during fibular osteotomy, and they explain the characteristic sensory loss over most of the dorsal foot with sparing of the first web space (deep fibular nerve) and lateral border (sural nerve). Clear surface relationships to the lateral malleolus and fibularis tendons help when planning regional anesthesia blocks and when interpreting post-surgical dysesthesia.
