Description
Arising from the tibial nerve in the tarsal tunnel, the lateral plantar nerve is traced distally across the plantar aspect of the midfoot, its course inferred through dorsal cutaway of the intrinsic muscle layers and adjacent skeletal landmarks. Metatarsals and phalanges anchor the relationships, while dorsal extensor tendons such as extensor digitorum longus and extensor hallucis longus run superficially and anterior to the tarsal and metatarsal bones. Medial to lateral, the nerve trajectory aligns toward the lateral forefoot, approaching the fifth metatarsal region and the digital branches to the lateral toes, with accompanying plantar vasculature indicated in blue. Deep anatomy, made legible. Clinically, the lateral plantar nerve is the workhorse of the lateral sole, supplying quadratus plantae, abductor digiti minimi, and the interossei through its deep branch, and providing sensory innervation to the lateral plantar skin and the lateral one and a half digits. Entrapment can occur at the level of the abductor hallucis fascia or along the course of the first branch (Baxter nerve), a common contributor to chronic medial heel pain that mimics plantar fasciitis but produces neurogenic symptoms and intrinsic muscle weakness. This dorsal perspective helps you reconcile the plantar nerve map with dorsal landmarks used during examination and when orienting incisions around the midfoot and forefoot.
