Description
Running superficially along the posterolateral distal leg, the sural nerve descends between the gastrocnemius and soleus region and then tracks inferiorly behind the lateral malleolus of the fibula before continuing onto the lateral border of the foot as the lateral dorsal cutaneous nerve. From this dorsal perspective, the nerve sits lateral to the calcaneal (Achilles) tendon and posterior to the fibularis (peroneus) tendons as they course toward the base of the fifth metatarsal, with the extensor tendons and dorsal fascia more anterior. Bony landmarks include the distal tibia and fibula, talus, calcaneus, cuboid, and the metatarsals and phalanges, giving clear reference for the nerve’s relation to the lateral malleolus. Surgeons and anesthetists care about this course because the sural nerve is a frequent donor for nerve grafting and is vulnerable during lateral ankle approaches, percutaneous Achilles procedures, and hindfoot endoscopy. It also provides a clean teaching example of cutaneous innervation on the dorsolateral foot, and why traction or scar tethering after lateral malleolar fractures or ankle sprains can leave persistent dysesthesia along the lateral foot.
