Description
From a medial perspective of the male foot and ankle, the lateral plantar nerve is traced after branching from the tibial nerve within the tarsal tunnel, deep to the flexor retinaculum and posterior to the medial malleolus. It courses anteriorly and laterally across the plantar aspect of the calcaneus, traveling between flexor digitorum brevis (superficial) and quadratus plantae (deep), then approaching the lateral compartment toward abductor digiti minimi. Adjacent plantar vasculature, including the lateral plantar artery and accompanying veins, runs in close company as the nerve sweeps toward the lateral sole. Orientation landmarks likely include the calcaneus, talus, distal tibia and fibula, and the medial longitudinal arch structures. Medial viewing clarifies relationships that matter in plantar heel and arch pain, because the lateral plantar nerve can be irritated at its origin or along its early course, including entrapment of the first branch (inferior calcaneal nerve, Baxter nerve) between the calcaneus and deep fascia near the abductor hallucis and quadratus plantae. This is a common mimic of plantar fasciitis and may also complicate medial approach procedures around the tarsal tunnel or calcaneal spur surgery. The curved lateral trajectory also explains why distal symptoms can present along the lateral forefoot while the compressive site remains proximal. Small structure, large consequences. Podiatric anatomy teaching, peripheral nerve blocks (tibial nerve and plantar nerve blocks), and operative planning for tarsal tunnel release benefit from this angle because it keeps the medial malleolus, flexor retinaculum, and plantar muscle planes in the same line of sight. It also fits well in journal figures or patient-facing education that distinguishes plantar fasciitis from Baxter neuropathy using clear anatomic landmarks.
