Description
Centered on the root of the mesentery, the superior mesenteric plexus forms a dense autonomic network wrapping the superior mesenteric artery as it courses inferiorly from the abdominal aorta, with fine perivascular fibers radiating along arterial branches toward the midgut. Posteriorly, the plexus lies anterior to the vertebral column and adjacent great vessels, while its ganglionated elements cluster near the origin of the superior mesenteric artery and the intestinal trunk. Sympathetic fibers from thoracic splanchnic nerves mingle with parasympathetic vagal contributions, then distribute distally toward the small intestine and the proximal large intestine. The colon is rendered with typical haustration and segmental contours, providing spatial context for autonomic supply to the ascending colon and proximal transverse colon. Posterior orientation matters because it aligns with how surgeons and interventionalists encounter these nerves around the superior mesenteric vessels during retroperitoneal exposure and mesenteric root workup. Injury, traction, or thermal spread near the superior mesenteric plexus during procedures such as right hemicolectomy, pancreaticoduodenectomy, or lymphadenectomy can produce postoperative dysmotility, secretory changes, and refractory diarrhea by disrupting sympathetic inhibition and visceral afferents. This is where anatomy stops being abstract. Ideal for teaching autonomic innervation of the midgut in gross anatomy, neuroanatomy, and GI surgery modules, and for illustrating discussions of mesenteric ischemia workups, SMA-first surgical approaches, or complications of extensive nodal clearance in oncologic resections.
