Description
Centered on the extrahepatic biliary tree, the gallbladder lies inferior to the liver and continues superiorly into the cystic duct, which joins the common hepatic duct to form the common bile duct that descends toward the duodenum. An anterior view places the cystic plexus as a fine network draped around the cystic duct and neck of the gallbladder, with fibers tracking medially toward the common hepatic duct and inferiorly along the common bile duct. Small periductal vessels and connective tissue planes are suggested along the hepatocystic triangle (Calot triangle), where the cystic artery typically crosses superior to the cystic duct. Spatial relationships matter here. For cholecystectomy, this is the nerve field that gets stretched, cauterized, or inadvertently divided while achieving the critical view of safety, and it helps explain why postoperative right upper quadrant discomfort can persist even after an uncomplicated dissection. The cystic plexus carries autonomic fibers (sympathetic via the celiac/hepatic plexuses and parasympathetic via the vagal trunks) that follow arterial routes to the gallbladder wall, so inflammation in acute cholecystitis can sensitize these pathways and produce referred pain to the epigastrium or right shoulder via associated diaphragmatic irritation. Surgeons also encounter variable periductal neural and vascular twigs that can obscure identification of the cystic duct–common hepatic duct junction. Use this anterior composition in hepatobiliary anatomy teaching, surgical atlases illustrating Calot triangle dissection, and patient-facing materials that need a clear depiction of how the cystic duct joins the common bile duct. It also suits radiology correlation when explaining MRCP or ERCP findings around the cystic duct insertion and common bile duct caliber.
