Description
Running inferiorly in the superior mediastinum, the superior vena cava is formed by the union of the right and left brachiocephalic veins, with the confluence positioned just to the right of the midline and posterior to the manubrium. From an anterior perspective, the vessel descends on the right side of the ascending aorta and anterior to the right pulmonary artery before entering the superior aspect of the right atrium. The right atrial appendage sits anterolateral to the caval entry, while the aortic arch and pulmonary trunk occupy a more leftward and central relationship in the same field. Coronary vessels course in the atrioventricular and interventricular grooves across the anterior surface. That anterior relationship is a working map for real procedures. Central venous catheters and pacer leads advanced from the internal jugular or subclavian veins should track through a brachiocephalic vein into the superior vena cava, with the cavoatrial junction targeted to reduce arrhythmia and perforation risk. SVC syndrome from mediastinal mass or thrombosis narrows this channel and forces collateral venous pathways, and this view helps explain why symptoms cluster in the face, neck, and upper limbs. Orientation matters.
