Description
Posteriorly, the sciatic nerve descends in the posterior thigh deep to the gluteal region and continues toward the popliteal fossa, where its tibial and common fibular (peroneal) components track inferiorly around the knee. Along the posterolateral calf, the sural nerve is shown coursing superficially in the distal leg, typically formed by the medial sural cutaneous nerve (from the tibial nerve) with a communicating branch from the common fibular nerve. Distally it passes posterior to the lateral malleolus and continues along the lateral margin of the foot toward the dorsolateral forefoot. Yellow nerve mapping contrasts with the femur, tibia, fibula, talus, and calcaneus to clarify depth and alignment. Posterior mapping of the sural nerve matters because it is a purely sensory nerve that clinicians intentionally target and just as intentionally try to avoid. A small structure, big consequences. The sural nerve is harvested for autologous nerve grafting, and it is at risk during posterolateral approaches to the ankle and hindfoot (including Achilles tendon procedures), as well as during endovenous ablation or stripping of the small saphenous vein where the nerve can run adjacent in the distal calf. This view also reinforces bedside localization for sural mononeuropathy, with numbness or dysesthesia along the posterolateral leg and lateral foot. Ideal for teaching lower-limb peripheral nerve anatomy in gross anatomy, neurology, and anesthesiology curricula, and for illustrating operative corridors in foot and ankle or peripheral nerve surgery texts where the relationship to the lateral malleolus and distal fibula must be clear.
