Description
Medial aspect of the talus (astragalus) is presented with the medial malleolar surface centered, the smooth comma shaped articular facet oriented superiorly to meet the tibial malleolus. Superiorly, the trochlea tali continues toward the talar dome, while inferiorly the talar body transitions toward the posterior process where the groove for the flexor hallucis longus would lie on the posterior aspect. Anterior to the body, the talar neck projects toward the head, which would articulate with the navicular in the talonavicular joint. Bone only. No soft tissues. This medial view matters because it isolates the tibiotalar contact area that drives ankle congruency and load transfer, a region commonly scrutinized in osteochondral lesions of the talar dome and in post traumatic malalignment after ankle fractures. Varus or valgus tilt of the talus under the tibial plafond often declares itself at the medial facet first, and that subtle change is what dictates joint preserving options versus arthrodesis in end stage ankle arthritis. It also maps the bony corridor used when orienting medial screws for talar neck fractures, where preserving the talar blood supply from branches of the posterior tibial artery remains a constant concern.
